Payment Policy

Insurance Billing: We bill most major insurance plans. Patients are responsible for co-pays, co-insurance, and any deductibles at the time of service. Please bring your insurance card to every appointment.

Self-Pay: Patients without insurance are required to pay in full at the time of service. We accept major credit cards, debit cards, and cash.

No Show and Cancellation Policy

Cancellations: If you need to cancel or reschedule your appointment, please inform us at least 24 hours in advance. This allows us to offer your slot to another patient in need of care.

No Shows: Patients who fail to show up for their scheduled appointment without a prior notice may be subject to a no-show fee. Repeated no-shows may result in limitation or termination of services.

Late Arrivals: Patients arriving late may have a reduced consultation time or may need to reschedule their appointment to ensure that we can provide the best care to all our patients.

Sliding Fee Discount Program
Chrisart Medical Inc offers a sliding fee schedule aimed at ensuring affordability of healthcare services, aligning with the federal poverty guidelines set by the U.S. government. The company is committed to ensuring that no patient is turned away due to financial constraints.
Eligibility for the Sliding Fee Discount Program is determined by assessing a patient’s household size and annual income. Applicants must complete an application and provide proof of income. Discounts are granted according to the federal poverty income guidelines, which are updated annually by the federal government.
While fees for eligible services are reduced, patients are responsible for a nominal fee for the service received, payable at the time of service.
Annually, both existing and new patients are required to provide proof of income*, a picture ID, and current income tax forms.
*Paystubs = 1 month | Tax returns = 1 year | Proof of no income = 3 months
